Dr Miller’s lips pursed in thought for a moment, and you could almost see the evil intent swirling behind her eyes before she seemed to reach a decision, mouth twisting into a smirk. “Yes, I think I’ll give you to Allira. She’s shown no obvious discomfort with our methods to date, so I’m sure she will be happy to help you.” Decision made, the doctor turned to the small laptop at the edge of her desk and began searching for a relevant file. “You’ll make a good training prop for that one as well. The girls always give extra special care to their first assignments you know. It’s an important relationship and they become very invested in making sure that you get all the care you need.”

You attempted to speak, but Miller just ignored you and continued, eyes now fixed on the screen. “You know Allira is quite the promising recruit to our little operation. She’s the kind of girl who sees your kind as cute little things in need of protection, but as far as our records indicate she’s never had direct interaction with one of you bugs that wasn’t being used as an object.” A small snort slipped from the older woman. “Well, I’ll never understand how these girls could have such compassion for worthless specks like you, but as long as they don’t really think of you as human it will work out.” You’d seen the kind of behaviour she was describing far too often, both directly and indirectly. Far omen who seemed to unintentionally fall into the trap of treating tinies like small children or pets.

While you dreaded your fate, Miller had started to type, accessing your profile and editing it in real-time. “Mr Woods suffers from intense MDPD, including intense dissociative episodes and noted incidents of memory loss. He has been unable to demonstrate any firm determination as to his most desire form, so he will need to be run through as many forms as possible to assist him.” She checked a note from the clipboard she had been holding when she first appeared. “Mr Woods has previously advised his referring psychologist, Dr Rachel Forest, on several occasions that he wished to serve as her undergarments, only to retract such statements at a later date. More recently he has experienced vivid hallucinations focused around Dr Forest’s rear, including making unfounded, and presumably hallucination induced, allegations that Dr Forest has been mistreating her husband.”

She clicked into another box. “If any concerns are raised by Mr Woods during treatment, particularly concerning his memory issues, he should be brought directly to Dr Miller for further assessment.” You gulped as Miller stared pointedly at you after reading this new information. It was clear that her intent was to make it clear to you that if you acted up around Allira you would just be brought back to her. The only reason you were even being made privy to her new notes was evidentially to strike fear into your heart and increase your psychological torment. Subjected to such sadistic cruelty, you could only wonder what made people like this cruel doctor despise tinies so much just for existing.

As you struggled to come to terms with this horrific reality, Miller leaned forward and pressed an intercom button near the far-edge of her desk. “This is Dr Miller. Could Nurse Allira please come to my office. Thank you.” Seeing a small chance you tried to rush the previously unseen intercom, but Miller just rolled her eyes and swept you off her desk with a callous swipe of her arm. After a brief period of being airborne, you crashed to the ground and found yourself lying in a partially squished heap before the office’s looming doorway. You hadn’t even managed to stand before the doorway was filled with the alluring form of your new ‘caregiver’. The redhead came bouncing into the room, the brief glimpse you caught of her face revealing obvious excitement and nervous anticipation.

By now you weren’t even surprised as the bronzed sole of the nurse’s left foot blotted out the office lights, leaving you just long enough to wonder briefly why she wasn’t wearing socks or shoes before it crashed down to squish you flat. While fit and slim outside of her monstrous curves, Allira remained a young woman of above average height and had to carry the weight of her generous assets, so she was more than heavy enough to crush you without effort, the majority of your body reduced to a smear beneath her sole while your head was battered and squelched by her powerful toes. The excited girl didn’t even notice the change in texture beneath her foot, predictably coming to a stop standing on top of you, just one eye able to peek out from a gap between her toes.

Though you were in no positive to appreciate it, Dr Miller did an admirable job of disguising her smirk as a smile when the small noise of your body being flattened reached her ears. “Hello Allira. As you’ve likely surmised, I’ve called you here because I’ve decided to make you the primary carer of Mr Woods. I trust I don’t have to remind you how important such a role is.” Her smile only widened as the young nurse practically vibrated in excitement, bouncing on her feet in obvious glee.

Allira was over the moon to hear that she was finally getting a tiny. She had anticipated this may happen when she’d been summoned so soon after you entered the doctor’s office, but had tried not to get her hopes up too much. “Yes! Ah, I mean… yes, I understand Dr Miller. Thank you for trusting me.” Attempting to remain still, the redhead scrunched her toes into the floor to ground herself, unknowingly squishing the features of your face as the digits played with the foreign object. “Umm… If I may ask, where is he now? I’d like to get started with helping him as soon as possible.”

Raising a single eyebrow, Miller glance at the edge of her desk before making a show of rising to her feet and leaning over the desk to examine the floor then pointing to the nurse’s feet. To her credit, Allira only started down in confusion for a moment before gasping and lifting her right foot to check beneath it, scanning the floor and her sole for any sign of her missing patient. Trapped beneath her left foot you felt the weight on you double and groaned internally as her toes dug deeper into the carpet to support the redhead’s weight. A moment later your burden was relieved as the foot you were attached to finally lifted up, hanging in the air momentarily before being flipped over as bright searching eyes narrowed momentarily before widening in recognition.

Face-to-face, you confirmed your earlier assessment that Allira was close to your own age, or maybe slightly younger, and of a thoroughly mixed-ethnicity with smooth ochre-hued skin and red hair that too crimson a hue to be anything but dyed, pulled into a long ponytail that hung down to the middle of her back. High cheekbones complimented her small nose, full lips and soft chestnut eyes which now gazed down at you in apology. Despite your best efforts, your low vantage point made it hard not to stare at the nurse’s most notable assets. You almost felt you should be used to such things by now, but the sheer girth of the girl’s rear and chest alike nearly took your breath away, and you could only wonder at what artifice of nature made it possible for such curves to appear so firm.

While you day-dreamed, momentarily forgetting your location and dire predicament, those dark lips parted, bathing you in a warm breeze as the massive nurse spoke. “I’m sorry Mr Woods. I did not see you there. I will-” She was cut off as Miller cleared her throat audibly, fixing the girl with a stern look. “That’s quite enough of that Nurse Allira. Treatment has now commenced, so I will not have you apologise for squishing your charge. Also, he is no longer Mr Woods. If you must refer to him by name, use his first name, though I would recommend using a pet name of your choosing.”

“Right!” Caught off-guard by the reprimand, Allira just barely caught herself before she could try to stand to attention and squish you again, taking a deep breath to steady herself instead. She recalled now the instruction to which Dr Miller referred. That the nurses should not apologise for squishing their patients and should, where practicable, not treat the patients as they would an ordinary person. It was all part of the process of helping them find their calling, unrestrained by societal expectations of reasonable conduct. Admonishing herself for the oversight, the bodacious girl finished peeling you from her sole with minimal effort and returned you to her superior’s desk.

Thoroughly smashed, you twitched painfully as you were finally released from Allira’s warm flesh, your body flopping uselessly as it struggled to reform. Shaking off your earlier distraction, you looked up into sparkling eyes that gazed down at you over the mass of your new caregiver’s prodigious chest, a contrast to the smirking face of Miller. Even now you could tell those eyes were sincere, but they also held a disconcerting intensity that chilled you to your core. Before you could gather the energy to speak her voice boomed out, restrained excitement obvious in her tone. “It’s so great to meet you again little one. I swear I’m going to do everything I can to make you better.”

Realising that things could go far worse if you didn’t make yourself heard, you forced your reforming throat to form words as you tried to rise on shaky limbs and face this oblivious girl. “Wait! I swear, I’m not unwell. Dr Miller is lying. I don’t have MDPD. I’m not sure that it even exists!” You could almost feel the doctor’s frown at you back, but you ignored it, staring up at Allira with all of the intensity you could muster, and watching in trepidation as her smile dipped and she bent down to get closer to you. The nurse’s chest strained at her scrubs and the bra hidden beneath, and her hot breath washed over you as lips parted to speak one more, revealing that…
