Well, at first you couldn't think of a wish to make. Getting sent into the world of an anime dating sim- or rather, the satire of an anime dating sim- seemed like more than you could ever hope for.
But the more you thought about it, the more you realized this was your chance to act on some of your... stranger desires. If you waited around and didn't use the wishstone now, it's power would likely be greatly diminished once you were finally transferred into the game world. Which meant that if you truly wanted to shake things up, this was your only chance...
And you did have some desires that you really, really wanted to see acted out. Not only that, but the world itself would change to suit your wish- such was the nature of starting your newgame+ with the wishstone's power already activated.
And deep down, you'd always had a secret fetish, a secret love of the good ol' cute monster girl trope. In particular, the bustier, thiccer, the better! And who was bustier than a cowgirl? That term was pretty broad, covering a fairly good-sized subgroups of monster girls, but the more you played the wish over in your head, the harder it was for you to get it out.
You didn't just want to be in the world of Harem Protagonist, you wanted to be in a busty, milk-flooded version of Harem Protagonist! Of course, you loved the heroines already, and you had no desire to change their personalities- something told you that the wishstone might not be able to do such a thing anyway.
